Use this form for appointment-only service requests in the South Bend / Elkhart / Michiana area. A submission is not an appointment, estimate, authorization, or approved work order.

Consent and spam protection

Cloudflare Turnstile is required for public submissions. The live deployment must configure PUBLIC_TURNSTILE_SITE_KEY; the server rejects submissions without a valid Turnstile token.

Server-side validation, Turnstile verification, D1 records, private R2 storage, and audit events must all succeed before Bigin sync is queued.

Appointment-only expectation: a website request is not an approved appointment, estimate, or work order until Crossroads Garage confirms it directly.
